Verify Your Coverage

It is the patient's responsibility to verify if we participate with your specific insurance carrier and plan. Please call the number on the BACK OF YOUR INSURANCE CARD prior to calling for an appointment.

Referral Requirements

If your insurance plan requires a Physical Therapy referral, it must be in place prior to your appointment. Please confirm this with your primary care physician before scheduling.

Co-Pays at Time of Service

Co-pays are required at the time of service. Please present your insurance card at every visit to ensure your claim is processed correctly.

Workers' Compensation

We participate in most Connecticut workers' compensation plans. When calling to schedule, please advise our office that this is a work-related injury. We will need a contact person at your place of employment.

ATTORNEY REFERRALS AND LETTERS OF PROTECTION (LOP)

Hat City Physical Therapy works with attorneys and accepts Letters of Protection (LOP) for patients who have been injured and are pursuing a legal claim. An LOP is a written agreement between your attorney and our office that guarantees payment for your treatment from the proceeds of your settlement or judgment.

This allows injured patients to receive the care they need right away, without waiting for insurance approval or paying out of pocket while their case is pending.

MOTOR VEHICLE ACCIDENTS (MVA)

Injured in a car accident? We treat whiplash, neck and back injuries, soft tissue damage, and other MVA-related conditions. We work directly with your attorney and accept LOP arrangements so you can begin treatment immediately.

WORKERS COMPENSATION INJURIES

Hurt on the job? We participate in most Connecticut workers compensation plans and coordinate directly with your employer and insurer. When scheduling, please let us know your injury is work-related so we can have the right information ready.
